This single field replaces m_iconized and m_maximizeOnShow which were
not really independent and will make it simpler to schedule either
maximizing or maximizing the window later, when it can't be done
immediately because the window is hidden, in the following commit.
Ensure that checking for dialog modality in wxEVT_INIT_DIALOG handler returns
true when the dialog is being shown modally in MSW and GTK.
Add a unit test checking that this is the case.

Windows CE doesn't seem to be supported by Microsoft any longer. Last CE
release was in early 2013 and the PocketPC and Smartphone targets supported by
wxWidgets are long gone.
The build files where already removed in an earlier cleanup this commit
removes all files, every #ifdef and all documentation regarding the Windows CE
support.

Modal dialogs shown during wxBusyCursor effect shouldn't show the busy cursor
as they do accept input, but did in wxMSW (as could be seen in e.g. the
widgets sample after enabling the "Global busy cursor" in the menu and showing
the text entry dialog via "Text|Set Help Hint").
Fix this by explicitly checking for the special case of having a modal dialog
as parent at wxWindow level as doing it in wxDialog simply didn't work as its
WM_SETCURSOR handler wasn't called at all for determining the cursor to use
for its children because they handled WM_SETCURSOR themselves in the global
busy state, without letting DefWindowProc(), which propagates this message
upwards the window hierarchy, to have it at all.

This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.
If nothing else, this will make an eventual transition to Git simpler.

Extract wxModalDialogHook from wx/testing.h into its own wx/modalhook.h,
extend it to allow to be notified not only about showing modal dialogs but
also about dismissing them and document it and show its use in the dialogs
sample.
Also replace all the WX_TESTING_SHOW_MODAL_HOOK macros occurrences with
WX_HOOK_MODAL_DIALOG.

Use the same short names as are used by the event table macros for the event
type constants themselves. This makes them much more comfortable to use, e.g.
Bind(wxEVT_BUTTON) compared to Bind(wxEVT_COMMAND_BUTTON_CLICKED).
The old long names are still kept for backwards compatibility and shouldn't be
removed as it doesn't really cost anything to continue providing them, but all
new event types should only use the short versions.

This allows to generate the menu open/close/highlight events correctly for the
popup menus used in the dialogs.
Extend the menu sample with a test using such menus.

Additionally renamed wxOSX' private wxNativePrinterDC::Ok() function to IsOk().
Didn't deprecate the various Ok() functions: given the amount of changes already introduced in 3.0 a trivial one like this seems more suitable for after 3.0.
