571d2e0f23 1. deprecate redundantly sounding wxArtProvider::FooProvider() to just Foo() 
2. wxArtProvider::Remove() now really only removes the provider without
   deleting it, Delete() does delete it
3. moreover, ~wxArtProvider removes the provider as well so there is almost
   no need to call Delete() explicitly any more

dee1a63ff5 Removed WXWIN_COMPATIBILITY_2_2 together with code guarded by it. 
Changed default for WXWIN_COMPATIBILITY_2_4 to 0.
  Added WXWIN_COMPATIBILITY_2_6 (defaults to 1).
  Ran build/update-setup-h.

532d575bd9 Allow MSW Base be usable in other ports than wxMSW (ie. wxTVision in windows console). Adjustements for limited setup.h (as in wxTV) and minor source cleaning.

ec6803d965 Some placeholder images to use until we get those that are being 
professionally done.  These fill out most of the art IDs that don't
currently have images in the standard provider.
