Handle internal idle (i.e. update handing)
*after* sending idle events so that if the
user calls Refresh() in OnIdle() the regions
will be updated afterwards.

INtroduced OnInternalIdle as per wxGTK so that users
cannot as easily lill the internals.
Fixed pop-up transient window.
Removed some #if 0 here and there.
Made refresh code work in idle instead of directly.

Lots of wxBitmap et al work.
Adapted wxIcon, wxCursor to this.
Split wxApp init things up so that the
X11 display is available sooner.
Changed initial display code accordingly.
Corrected wxDC::DrawBitmap.

accessor function that initializes if on first use. This prevents
core dumps for apps that try to create wxBitmaps before the wxApp
object is initialized.

gtk+ 1.3.1's widgets (except the deprecated/obsoleted ones) work
solely with UTF-8. (I knew my forward thinking regarding wxConvCurrent
would pay off someday...)

Changed refresh order so that window-less widgets
don't get overdrawn anymore,
Prevent GTK from filtering our supposedly unneeded
expose events,
Block all expose calls if a resize is requested and
do a full redraw then, thus removing flicker and
behaving like wxMSW,
Moved some doubled expose and draw code from GtkPizza
to the respective callbacks in window.cpp,
Set idle priority to a higher value (which should
indicate a lower priority if I interpret glib.h
correctly) but the desired side-effect didn't
really show up.
