3f66f6a5b3 Remove all lines containing cvs/svn "$Id$" keyword. 
This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.

If nothing else, this will make an eventual transition to Git simpler.

d44d80da5b Add wx/setup.h header for wxGTK build under MSW. 
This is identical to wx/msw/setup0.h but will be used by wxGTK builds using
MSVC when this is supported.

Closes #14395.

bd362275b8 Remove Palm OS port. 
The platform targeted by this port doesn't exist any more and the port never
achieved really working state so remove the code to avoid having to maintain
it.

508d12b224 extracted MSW-specific setup.h options to their own wx/msw/setup_inc.h file which is now used to automaitcally generate both wx/msw/setup0.h and setup.h.in by update-setup-h

8d51d7d2f9 1. set all wxUSE_XXXs to 0 in the generated setup.h.in 
2. don't update the files unless necessary (i.e. check modification time)
