c1f7d193f6 CocoaRemoveFromParent can (and will) be called when !m_cocoaNSView. 
However, if there is m_dummyNSView then there must be m_cocoaNSView.
Therefore, moved wxASSERT(m_cocoaNSView) inside if(m_dummyNSView)

93e73c740e Do not derive wxCursor from wxBitmap, plus misc cleanup. 
Derive wxGauge from wxGaugeBase, use XmScale for the
implementation if the Motif version supports the
XmTHERMOMETER style.
  Derive wxSlider from wxSliderBase; generate
wxEVT_SCROLL_THUMBRELEASE in addition to wxEVT_SCROLL_THUMBTRACK;
misc cleanup.
