8f8d58d193 Use wx-prefixed macros throughout the repository. 
Change {DECLARE,IMPLEMENT}_*CLASS and {DECLARE,BEGIN,END}_EVENT_TABLE
occurrences to use the wx-prefixed version of the macros.

3f66f6a5b3 Remove all lines containing cvs/svn "$Id$" keyword. 
This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.

If nothing else, this will make an eventual transition to Git simpler.

Closes #14487.

a1b806b982 Replaced Ok() occurrences with IsOk() throughout trunk. 
Additionally renamed wxOSX' private wxNativePrinterDC::Ok() function to IsOk().

Didn't deprecate the various Ok() functions: given the amount of changes already introduced in 3.0 a trivial one like this seems more suitable for after 3.0.

66f7556189 Compilation fixes for wx{X11,GTK1,Motif} after making ref data non copyable. 
Don't use wrong compiler-generated copy ctors in the other ports neither (see
r66371..66373 for wxGTK fixes). This fixes compilation after r66374 without
reducing functionality but just making it more clear where it is missing.

a0739a9a5e Fix harmless unused parameter warnings in wxX11 wxCursor. 
Parameters in wxCursor ctor are unused because it is not implemented but this
is already indicated by a wxFAIL_MSG() in it.

55ccdb93e4 COW cleanup patch (1583966): 
1. perform deep comparison for the classes for which it makes sense in the
   ports where this wasn't done yet
2. remove (shallow) comparison operators for the classes for which it does
   not make sense (such as wxBitmap)
3. makes wxBitmap use COW on all ports
4. adds wxObject::IsRefTo()
5. centralizes and improves COW docs

a11672a469 wxX11: 
Lots of wxBitmap et al work.
     Adapted wxIcon, wxCursor to this.
     Split wxApp init things up so that the
       X11 display is available sooner.
     Changed initial display code accordingly.
